A secondary claim is a condition caused or worsened by one the VA has already service-connected. The causation fight is already half-won — the VA agreed your diabetes or hypertension came from service; the question is only whether it caused the kidney disease. That's a medical question with a recognized answer.

The connection

Diabetic nephropathy and hypertensive kidney damage are two of the most documented cause-and-effect relationships in medicine — your own lab work (creatinine, eGFR, microalbumin) is the evidence trail. For veterans with service-connected diabetes or hypertension, later kidney findings should always be evaluated as a secondary claim; renal ratings key directly off the lab bands.

How the VA rates kidney disease

Renal dysfunction rates on kidney function (eGFR): 30% = eGFR 45–59 · 60% = eGFR 30–44 · 80% = eGFR 15–29 · 100% = dialysis or eGFR under 15. Kidney stones rate separately (7508-09). Bad labs ARE the rating — many vets are told "we'll watch it" and never told it's ratable.

What the claim needs — the three legs

  1. The anchor: service connection for the diabetes or hypertension (see how that condition is rated). Not connected yet? File it first — a secondary claim needs something to attach to.
  2. A current diagnosis of kidney disease — the VA rates diagnosed conditions, not symptoms.
  3. The medical link. An opinion stating it is "at least as likely as not" that the diabetes or hypertension caused or aggravated the kidney disease. The VA's own C&P examiner supplies this free in most cases; your own doctor can too. A paid independent opinion is the last resort, not the first move.
